Ontario Perinatal Record (OPR)

The Ontario Perinatal Record (OPR) replaces the Ontario Antenatal Record 1 and 2. Please order the new OPR and destroy remaining Antenatal 1 and 2 forms. The updated OPR is the fifth revision and replaces the 2005 version.

The new OPR and the development of the accompanying User Guide was undertaken in 2017 by a provincial committee representing practitioners using the current antenatal record, including obstetricians, midwives, family physicians, nurses and nurse practitioners, as well as other stakeholders that support high-quality maternity care such as Best Start, Public Health Ontario, BORN Ontario and PCMCH.

PCMCH is updating the OPR form and User Guide in 2024. This is being done in collaboration with partner organizations and experts from all perinatal care provider disciplines who use the form (i.e., midwifery, obstetrics, family medicine and nursing). 

To order:

Visit the Ministry of Health’s Central Forms Repository website.

In the search bar, enter “Forms Order Request” (form # 014-0350-93). On the request form, you will find the OPR listed (form number: 5046-64 “Ontario Perinatal Record set”) which you can select to order and submit for processing. The form is available to order in English (form number: 5046-64E) and French (form number: 5046-64F).

When ordering the OPR, there is now only one form that needs to be ordered, not two separate forms like the previous Antenatal 1 and 2. Extra copies of page 3 are available if you require more room to record subsequent visits (form number: 5060-64 “Perinatal Record 3″).
