Milestones are more than markers of time — they are meaningful reflections of growth and impact. Whether celebrating a birth, a graduation, or a program anniversary, milestones offer a moment to pause, recognize progress and envision what’s next.
This year, we celebrate a particularly important milestone: the 10th anniversary of PCMCH’s Complex Care for Kids Ontario (CCKO) program.
CCKO was launched in 2015 to improve the lives of children with medical complexity and their families by providing integrated, coordinated care across health and social systems and facilitating care closer to home. In this special issue of Milestones, you will read about the evolution and significance of the CCKO program. You’ll also hear insights from two CCKO family advisors, whose contributions ensure that CCKO remains responsive, patient- and family-centred. Finally, learn how Ontario’s regional maternal-child health networks are using CCKO resources like the Youth Transition to Adult Care Toolkit to improve care transitions and build bridges between paediatric and adult systems.
CCKO is an example of how PCMCH brings partners together to drive forward meaningful change. From the Emergency Department Paediatric Readiness Initiative to the Ontario Paediatric Bariatric Network, our programs work together to improve health outcomes and experiences for Ontarians. As always, we are deeply grateful to our partners for their ongoing collaboration and dedication to improving pregnancy, newborn, child and youth health.
