The CCKO (Complex Care for Kids Ontario) Youth Transition to Adult Care Toolkit resource, developed by the Provincial Council for Maternal and Child Health (PCMCH) in 2022, outlines the process for children and youth with medical complexity (CYMC) transitioning from the CCKO Program to Ontario’s adult care system. The objective is to streamline tasks for Healthcare Providers (HCPs) and Caregivers/Youth by providing guidance through age-based checklists and a Transition Resource Guide that highlights key services and funding supports available for this unique paediatric population. The age-based tasks in both checklists are nearly identical to ensure a concerted and collaborative approach. Download the individual tools below.
