PCMCH works closely with frontline healthcare providers to improve the health and well-being of women and newborns in Ontario. We build smart partnerships with local experts who really know and understand your healthcare needs to create a strong and sustainable maternal-newborn system of care that supports mom, baby and family throughout pregnancy, childbirth and the postpartum period, thus helping to ensure the best possible beginning for all.
Supporting and influencing the healthcare needs of mothers, newborns and children, and where required, supporting children’s transitions to adult care, continues to be a key focus of PCMCH.
PCMCH is committed to working with local healthcare providers to ensure that the full spectrum of children’s services is available to children and their families, regardless of where in Ontario they access care. This includes building a more equitable access to care as close to home as possible for children who are medically fragile and/or technology dependent, providing coordination for 35 Ontario specialized paediatric diabetes programs to ensure the best possible health outcomes for children and youth affected by diabetes, and developing effective solutions to allow children and their families an opportunity to have a safe and healthy transition into adult healthcare system.
